1-RESPONSABILITIES AND TASKS

  • Community Health Interventions

 

  • As part of the Community Health Intervention the Nurse is part of the community health network and provides assistance through preventative services at community level.
  • The Nurse will support training modules for building the capacity of the community health workers - and other assigned staff - in their area of expertise/topic (such as behavioral change for non-communicable diseases, communicable diseases or any other topic of Public Health relevance)
  • Liaise with community health workers for the intervention development of outreach activities.
  • All the times the nurse refers the detected at risk beneficiaries to the nearest PHC Centers to assess full package of medical care and explain provision of services available at the PHCC level.
  • Support the development of educational material (e.g. field assessment/testing) for assigned topics of public health impact.

 

  • Community Outreach

 

  • Assess, keep the track and follow up on targeted beneficiaries with NCDs or other public health problem of concern such as communicable diseases in Informal Settlements and Host Communities in the supported PHCCs catchment area, or other selected priority areas where intervention is needed.
  • Provide basic preventative nursing checkup services to identified NCDs beneficieries during outreach activities: measuring blood pressure, blood glucose, weight, or other relevant information requested.
  • Ensure the beneficiary is taking the medication/drugs as prescribed by physician.
  • Provide information of supported hospitals for critically ill chronic patients.
  • In agreement and with previously informed consent of the beneficiaries, set up a visit plan for developing the health education program and counseling for NCDs.
  • Provide health education about the importance of visiting the PHCC, services provided and follow up on instructions provided by physicians in the PHCC.
  • Provide health education for any other assign population at risk to develop a NCD or any public health relevant communicable disease.
  • Liaise with the Health Field Officer, Physicians to keep track/follow up on special cases.
  • Referrals

 

  • Provide to all assessed beneficiaries information on PHCC services provision.
  • Follow up on referred cases with PHCCs to ensure patients access to adequate healthcare.
  • Ensure awareness and knowledge of referral options and pathways.
  • Closely follow up and monitor cases referred to PHC or secondary health care centers.
  • Closely follow up on cases facing difficulties in accessing PHCCs:  beneficiaries with special conditions such as physical disability, legal constraints/lack of documentation among others.
  • Link with the CHWs and follow up on identified cases.
  • Reporting

 

  • Collect necessary information from CHWs on identified NCDs and other conditions of Public Health relevance in communicable diseases and report accordingly.
  • Perform assessment to beneficiaries and report to the immediate data collected superior in a daily, weekly basis and compile reports on a monthly and quarterly basis.
  • Liaise with the IM Officer to make sure data registration is accurate on hard and digital copies.
  • Keep up to date the first line manager about changes observed in the field to ensure continuous learning process and activities adjustment.

Mandatory requirements

Language skills: Fluent in Arabic (English or French is an asset)

Education degree: Nursing Diploma, certified by the Ministry of Public Health in Lebanon

Work experience: Proven experience working as a nurse; preferably experience working in NGOs.

Knowledge and skills:

  • Good knowledge of  Lebanon governorate geography, willingness  to work at the community level
  • Excellent knowledge of counseling pregnant and lactating women
  • Excellent communication (especially in public) and diplomacy skills to manage relationship in potentially tense situations
  • Literacy in reporting, experience in conducting surveys

Computer skills: Excellent knowledge of the MS office software including Word, Excel, Outlook

Other: Valid driving license (1 year minimum)
