Roles and Responsibilities:

                                                                          

General duties and responsibilities:

 

  • provide comprehensive case management to persons with specific needs with high risk levels, including home visits

  • To ensure vulnerable individuals are identified and receive individual case management support in a timely fashion by conducting documentation, assessments, action plans, direct service provision, referrals and follow ups for an assigned case load
  • To provide support to vulnerable individuals with protection concerns in the process of psychosocial support and case management.
  • To work directly with other caseworkers and service providers on management of caseload and referral

Case Management:

 

  • Participate in supporting the identification of individual cases identified by the team or referred by other agencies.
  • Conduct psychosocial assessment of vulnerable cases, including environmental, emotional, behavioral, and social factors as well as resources and strengths that impact the individual well-being.
  • Plan with the Senior Social Worker/Social Work Manager to conduct home visits to assess the living conditions of the cases.
  • Ensure the preparation of accurate case plans for high-risk and vulnerable cases identified with clear outcomes and time-frame.
  • Advocate with different institutions and services providers to help beneficiaries to access needed resources and services.
  • Collaborate with the medical focal point for all medical cases
  •  Ensure coordination with other service providers in collaboration with the supervisor in the area of intervention for prompt referral, follow up and share information.
  • Manage, fill, and archive specific forms for profile cases, referral and report weekly to ensure confidentiality, accuracy and timeliness.
  • Monitor and follow up the outcome of the assistance provided.
  • Be available to provide basic emotional support for urgent cases.
  • Ensure the provision of emergency cash assistance for the most vulnerable clients, according to the needs of the clients, the agreed criteria and the internal procedures and under the supervision of the Social Work Manager/Protection Coordinator.
  • Participate in hotline management.
  • Participate in weekly casework allocation and planning meetings.
  • Ensure that data protection protocols are respected.
  • Available to participate in meetings and training inside and outside the center including case management meetings, case conference meetings or any other meetings when required.
  • Communicate regularly with the Senior Social Worker/Social Work Manager for updates on achievements and challenges
  • Accomplish any other assignments allocated by line managers. 
  • Provide comprehensive case management to people with specific need with high risks levels.
  • Conduct home visits, offer counselling, and put in place case management. Plan, track and follow the interagency SOPs.

 

 

Psychosocial activities:

 

  • Participate and collaborate in recreational, psychosocial, awareness activities conducted inside or outside the center when requested by the Social Work Manager.
  • Support the organization of specific events or activities inside or outside the center.

 

Monitoring and Reporting:

 

  • Provide daily and weekly updates to the Social Work Manager on cases assessed and supported.
  • Support the Senior Social Worker in the production of daily and weekly reports to the Social Work Manager, Protection Coordinator and Unit Coordinator.
  • Contribute to the update of the Makhzoumi Foundation case management system and UNHCR RAIS
  • Assist with screening work.
  • Assist for training new staff and provide feedback
  • Assist in Validation Exercises and Distribution, monitoring and protection desk

 

Individual Case Management:

 

  • Identify and/or receive individuals and families (including careers) with specific needs who are detected during registration, psychosocial, outreach and other activities by the team or referred by other agencies.
  • Review and/or conduct a risk assessment and prioritization for intervention.
  • Conduct vulnerability assessments and household visits to assess the needs of individuals and families.
  • Provide information to individuals and families about their rights and entitlements, including what services and assistance is available (including beyond the scope of services provided by AGENCY).
  • Offer individuals and families choices and options, and actively involve them in decision-making that will affect them.
  • Work with identified individuals and families to develop and implement the case plan in accordance with their needs, capacities and goals.
  • Making sure all options are explored before deciding on a plan. Looking for options when the services needed are not available.
  • Working with individuals and families to develop and/or maintain resilience. Linking individuals and families with support groups and networks, and other community-based resources, as appropriate.
  • Provide psychosocial counselling and case management services, including the coordination of provision of services by other specialized service providers in accordance with the needs of individuals and families. 
  • Ensure appropriate monitoring, review and follow up of the services provided to cases as per the standard operating procedures. Maintain a list of persons with specific needs for follow up and track their progress.
  • Ensure proper and updated documentation related to each individual case.
  • Determine if and when it is appropriate to discontinue follow up to individuals and families (case closure).
  • Be accountable to individuals, families and communities for the services they provide.

 

 

 

 

Psychosocial activities in community-based safe spaces:

 

  • Facilitate structured group sessions as part of carrying out activities, or evaluating specific activities, etc.
  • Participate and collaborate in other activities (recreational, psychosocial, awareness and non-formal education) when requested by the supervisor.
  • Support in organizing specific events or activities.

 

Data management and reporting:

 

  • Manage, file and store data ensuring the confidentiality of the information collected and the services provided according to the case management SOPs.  
  • Prepare and submit weekly, monthly and 3-month work plans in a timely manner.
  • Support the implementation of monitoring and evaluation tools and report on problems in the implementation of the program.
  • Participate in weekly reporting and planning meetings to ensure adherence to standards of care, quality case management and reporting requirements.

 

Coordination and representation:

                                                                                                       

  • Contribute to the maintenance of an up-to-date service mapping of the service providers operating in the area and take proactive steps to remain informed about the available services.
  • Advocate with institutions, community and religious groups/leaders and other service providers to help beneficiaries access needed resources and services as appropriate and after consultation with the supervisor.
  • Support individuals and families to effectively represent their views, needs and capacities in all meetings affecting them.
  • Participate and/or lead relevant meetings, as determined by the supervisor. 
  • Escort vulnerable cases to their shelters or to mental health facilities and others
