Summary and Background:

The Disability officer (DO) is the main focal point for coordinating and monitoring referrals and service provision within his area. He/she is responsible for coordination of care pathways from community settings to service delivery centers for people with a disability who have complex health care needs. The DO will have a thorough and service-oriented and beneficiary-centered approach and ensure that the project is implemented in accordance with recognized standards and practices, while continually works towards improving the impact of its services on PWD.

 

Place in the Organization

The DO position falls under the technical supervision of the Disability Advisor and the line management of the Health Area Manager. The DO will supervise all aspects of coordination and referrals between community health team, PHCC’s and service delivery Centers.

 

Functions and responsibilities

In accordance with established project guidelines, the DO will be the focal point in ensuring that target population receives intended services in the assigned area. S/he will:

  1. Receive assessments from community health workers and coordinates appointments and referrals to different service delivery centers (PHCC’S, rehabilitation centers, diagnostic centers) as needed.
  2. Coordinate with medical service providers at different clinics in the area for referrals and appointments for service delivery.
  3. Act as focal point, in coordination with medical advisors, for the approval of diagnostics and referrals for specialized diagnostics/aids/treatment.
  4. Supervise community health workers schedule and activities and provide on-the-job coaching and one-on-one trainings.
  5. Track community-based referrals and follow up system and report on percentage of positive referrals to ensure effectiveness.
  6. Monitor referrals and service delivery from the field and reports on findings.
  7. Work closely with PHCC health professionals and with community health workers to adapt care and referrals based on feedback from community.
  8. Conduct monitoring visits to PHCCs and contracted referral centers.
  9. Monitor service delivery quality in contracted referral centers (diagnostics/rehabilitation centers) and reports challenges.
  10. Follow up on data entry inside PHCCs and monitor quality and accuracy of data entered. Provide support for data entry clerks.
  11. Monitor and report any challenges and/or gaps identified to inform adjustments to the project.
  12. Complete project documentation for assigned activities. Assist with identifying information for case studies and reports on promising practices.
  13. Participate in meetings with Area Manager and community health workers to track indicators and prepare weekly reports on PWD project.
